User Name: IF YOU DO NOT HAVE A DISTRICT EMAIL ADDRESS please enter your lastname_firstinitial (e.g. smith_j for John Smith)
Password: IF YOU DO NOT HAVE A DISTRICT EMAIL ADDRESS please enter your four digit birthdate (e.g. 0225 for February 25)
